Thank you for your interest in adopting a Permes cat! If you are ready to apply to welcome a new kitten into your life, please fill-out our adoption questionnaire for families, found below.

What would you do if your new feline doesn't get along with your present/future pets, or current pets don't get along with the feline?

Have you considered expense, time and commitment required to care for and own a feline?

How many hours worked per week?

How many hours per day do you plan to spend with the new kitten?

How many hours per day will your new feline spend alone or with others on a regular basis?

Is anyone home during the day? If yes, who?

Where would a new feline be kept in your presence/absence?

If your present relationship/life were to change due to death, divorce, separation, loss of job, illness, etc. with whom will the cat reside with?

The feline's Adoption Contract states Breeder needs to be notified of changes. Do you object?

Your Kitten's/Cat's Future

Where would the feline sleep at night?

Do you plan to have litter boxes in more than one room for the kitten?

When you are not there, such as stay in the hospital, go on vacation, or visit for a few days, who will care for the feline?

Your new feline could take up to two weeks or longer to adjust to your home. Are you willing to allow time to adjust and contact Breeder with questions or concerns?

Would you object to Breeder inquiring/following up with calls and/or home visits to check on feline's progress if within a reasonable distance between our homes?

Will the new feline be allowed on your furniture, table or counters?

If no, why? What do you plan to do to discourage this behavior?

What is the name, address and phone number of your current veterinarian or do you have a veterinarian lined up as your vet of choice for your new kitten?

Are you familiar with the feline diseases and know the importance to stay current on immunizations if you should decide to make your cat an indoor/outdoor cat?

Feline Viral Rhinotracheitis (FVR), Feline Calicivirus (FCV), Feline Panleukopenia (FP), Rabies, Feline Aids (FIV), Feline Leukemia (FeLV), and Feline Infectious Peritonitis (FIP)?

(This concludes the questions on our questionnaire. Follow up questions may be necessary.)

By submitting this questionnaire, you certify and acknowledge that all information provided is true and correct. All deposits are non-refundable unless the breeder is unable to provide you with your kitten of choice as agreed or due to illness.

PS All my adult cats are DNA tested for genetic abnormalities or defects to ensure I minimize producing cats with genetic defects. I also send them to their new homes fully vaccinated, microchipped, examined by a licensed Veterinarian and I run an Elisa Comprehensive Fecal test to make sure my kittens are Healthy and Sound before they leave my home. They are also sold on a Sales Contract and a I guarantee their health. Thank you once again for choosing us for your next kitten.
